Insurance
As a service to our patients
we will complete and submit insurance forms to your
insurance carrier. It is imperative that we maintain
accurate insurance information. Please notify our staff
of any changes in your insurance and personal demographic
information to maintain accuracy in our billing, as
well as to ensure that we are able to reach you by mail
and telephone, if necessary. Our physicians participate

Billing
As participating physicians we
will adjust the amounts that are considered non-allowed
by your insurance carrier. However, you will still be
responsible for the percentage of allowed amounts your
policy does not cover, as well as co-payment and deductible
amounts. If you have questions regarding whether we
accept or participate with your insurance, please call
(256) 533-4402 and follow the prompts for our Insurance/Billing
Department. Or, you may request billing information
and inquire account balances or statement summeries
by emailing cfnbilling@clinicforneurology.com.
All office visit co-payments
and coinsurance amounts are due at the time of service.
We accept VISA, MasterCard and Discover for your convenience.
